How To Apply

Children ages 18 and younger should be referred to us by a medical, school, or social services professional who is very familiar with their case.  Because funding is limited and we aim to assist those with the most need, we do not accept self-referrals.  In order to be good stewards of our funds, financial need must be documented.

We accept referrals from:

  • Foster Care Case Worker

  • Guardian Ad Litem

  • Early Interventionist/Baby Net Provider

  • Health Care Provider (doctor, nurse, nurse case manager, nurse practitioner, social worker, child life specialist, therapist)

  • School Professional (teacher, guidance counselor, principal, assistant principal, school therapist, school psychologist)

  • Family Connection
